Fitness for Work Certificates
You can request a fit note from your surgery. You can do this via our "Contact Us Online (AccuRx)" service, which is on the main home page of our website or by telephone or email. Your request will be processed by our reception or admin team who will send your request to your named GP. Should your GP need to see you, you will be contacted by the surgery to arrange an appointment.
